The Palliative Care Initiative (G14063, G14069) helps GPs provide compassionate, collaborative, and holistic care to palliative patients by enabling them to take the required time to plan and coordinate end-of-life care for their patients.

The initiative supports the work of family physicians making the various decisions and plans that need to be determined to ensure the best possible quality of life for dying patients and their families.

This incentive compensates the family physician for:

• undertaking and documenting a care plan, and
• clinical follow-up management of the patient.
