Patient Conferencing Initiative

The Patient Conferencing Initiative (14015, 14016, 14017, 14018, 14021, 14022, 14023) compensates GPs for case conferencing with other health care providers (e.g., specialists, specialty care nurses) in the treatment of patients with complex, mental health, and palliative conditions, as well as the frail and elderly.

These payments are for case conferencing for complex patients who are facility-based, community-based, or ready for discharge from a hospital. They are limited to the care of BC patients (out of province patients are not eligible) who fall into five categories:

  • frail elderly
  • palliative care
  • end of life
  • mental illness
  • patients of any age with multiple medical needs or complex co-morbidity (two or more distinct but potentially interacting problems where care needs to be coordinated over time between several health disciplines)

The acute care discharge planning conference fee may be billed for patients who need support from a multi-disciplinary team or who need to transition from an acute care facility to the community or another facility.
